Root Cause Theory:

There is a theory that if we all work hard toward goals, we can continue to improve current processes. This is known as the fallacy of continuous improvement.

I any product, project or process was given EVERYTHING needed in the way of resources, people, capital and time, it still would only be able to deliver to a certain level. That level of production or capability is known as "entitlement." Any expectation over the point of entitlement is simply fallacy. Even attaining entitlement is almost impossible as any process or product will not have the ability to maintain the level of perfection within the process to deliver to that theoretical top performance. People are sick, products aren't perfect, sub components do not always meet spec, etc. etc.

So when looking for the root cause, consider the fallacy of continuous improvement and spend some time understanding the capabilities of your project. Sometimes good enough simply is "good enough."
